Background
South Africa has developed a National Cervical Cancer Elimination Strategy aligned to the WHO 90-70-90 targets. While policy direction is established and implementation has commenced, sustainable national scale-up requires a robust economic and fiscal justification to support:
- National Treasury engagement
- Medium-Term Expenditure Framework (MTEF) planning
- Provincial budget alignment
- Donor and blended financing discussions
- Long-term sustainability beyond external grant funding
The Investment Case will provide a rigorous economic, fiscal, and health impact analysis comparing elimination scale-up scenarios against the status quo, quantifying the return on investment and affordability within the current health financing environment.
Purpose
To develop a comprehensive, evidence-based National Investment Case that demonstrates the economic, fiscal, and societal value of scaling cervical cancer elimination in South Africa and provides a clear financing and implementation roadmap.
Objectives
The Investment Case will:
- Quantify the total resource requirement to achieve elimination targets.
- Model health impact (cases averted, deaths averted, DALYs/QALYs gained).
- Assess cost-effectiveness (ICER per DALY/QALY).
- Estimate return on investment (ROI) and long-term fiscal savings.
- Conduct affordability and budget impact analysis aligned to MTEF cycles.
- Propose sustainable financing options and phased scale-up scenarios.
- Identify key implementation risks and mitigation strategies.
